1. How to Request Your Chiropractic Records

Patients of Weber Chiropractic have the right to access and receive copies of their health care records.

To submit a request:

Method: Send a written request via email to frontdesk@weber-chiropractic.com.

Requirements: Please provide your full legal name, date of birth, and specific dates of service requested.

Processing: Records are provided in accordance with Texas law and HIPAA regulations. Standard fees for duplication and postage may apply.
